Inlay, Onlay, Filling, or Crown? Choosing the Right Restoration

When to Use Each Restoration

Direct Filling (Composite)

  • When: Small cavity (less than 1/3 of chewing surface)
  • Lifespan: 5-10 years
  • One session

Inlay

  • When: Medium cavity within cusp boundaries
  • Lifespan: 15-25 years
  • Made in the lab from porcelain or zirconia

Onlay

  • When: Larger cavity involving one or more cusps
  • Lifespan: 15-25 years
  • Preserves more tooth structure than a crown

Crown

  • When: Major tooth damage (more than 2/3)
  • Lifespan: 15-20 years
  • Covers the entire tooth

The Golden Rule

The more natural tooth structure we preserve, the better the long-term result. That is why we prefer filling → inlay → onlay → crown (from least to most invasive).
